NEXT GENERATION SPECIALTY CARE
Physiatry-Pharmacy Collaborative (PPC)
To help patients take better care of themselves at home, NEW self-management prescriptions need to be dispensed. Our targeted “prescription” focuses on optimizing a patient’s mobility, medications, and mentation, in order to help them achieve the goals that are most important to them. With PPC, patients are supported with a tech and team based approach to enhance primary health care outcomes.
